I'm a little confused about pen names. I assume you use your real name when giving contact details and your pen name when saying who the novel has been written by, but which do you put in the heading that appears at the top of each page?

One method of pen name use in headings is this, flush right:

X is the page number.
TITLE is a one-word version of your title, in caps
Name is your real last name
PenName is your pseudonym's last name.

If it's a novel, your title/cover sheet follows standard format for such pages, with "by PenName" centered but your real name and contact info up top.
